Holiday Extravaganza Booth Application

The 5th annual holiday extravaganza will be held on December 1st from 4 to 8 pm. 

Booth spaces are $25 each and come with 1 6ft table and 2 chairs. Additional tables are $25 each.

You may fill in an appliation below and mail with payment (see address below).

We do not duplicate vendors of companies such as Avon, Mary Kay, Siplada, etc.  As vendors sign up for this event we will post a list below.
