Citizen of the Year Nomination Form

Please fill out the form below to nominate your choice for Mattawan's Citizen of the Year for 2011, all lines must be completed. 

Rules:

• Nominee must be a resident of the Mattawan School District
• Nominee must be willing to accept the recognition of citizen of the year
• Nomination must be submitted no later than October 10, 2011
• Be as descriptive as possible why you are nominating this person for citizen of the year (voters will be reading this info)

After October 10th

All nominations will be put on online voting form with nominees name and why they are being nominated.  Voting will occur on October 10th and the nominee with the most votes will be honored at the dinner.
