Citizen of the year nomination form

Who will be Mattawan's next shining star?  You can decide that here by submitting a nomination for your choice of "Mattawan Citizen of the Year".

This year at the 2011 Mattawan Business Association Fall Appreciation Dinner we will be honoring community volunteers, business owners and one citizen of the year. 

How does it all work?

The rules are simple:

• Fill out this nomination form as completely as possible and tell us just why the person you are nominating should be recognized as citizen of the year. 
• Nominees must reside within the Mattawan School District.
• Nominations must be submitted by August 24, 2011. 

What comes next?

• The person with the most nominations will be honored at the 2011 appreciation dinner with a plaque presented at our awards ceremony.
